Job Description:
- Tasks and Responsibilities:
Supervise and assist in out Pharmacy Services inventory management; collect information requiredfor interventions. Review, monitor and control drug profile and Computerized Physician’s Order Entry (CPOE) of allout door patients, Contact with clinical pharmacist and physicians in case of in-accuracy of order /non availability of medicine or alternative. Assist to clinical pharmacist in Antibiotic stewardship round to explore the role of Pharmacisttowards patient care. Assist to clinical pharmacist and monitor high alert medication dispensation and provide all relatedprecautions and information for proper administration. Maintain document all interventions on the system, and present all relevant issues to clinicalpharmacist, Pharmacy Services daily. Compile and prepare presentations/ data on Effective Interventions monthly. Assist pharmacist, Pharmacy Services to develop hospital formulary and ensure its effectiveimplementation at TIH. Develop strong communication between the Pharmacies, clinical and nursing staff Identify and correct HMIS related problems (in coordination with HI), and provide summaryreports to clinical pharmacist. Review and monitor stock of all crash trolley medicines and present monthly reports to clinicalpharmacist. Assist to pharmacist to Provide current and accurate drug information to health care professionals.Drug information would include a thorough understanding of new medications, and suitableformulary agents for use in specific therapies.Assist to pharmacist to Record drug information data and prepare graphical reports of monthlydrug information, provided by the Pharmacy department. Provide technical and administrative support to Pharmacists, Pharmacy Technicians and training tothe interns. Monitor and record all non-formulary drugs available at site. Perform all related tasks assigned by line Manager Pharmacy/ Assistant Manager Pharmacy.
