Eligibility / Qualification Required:
About the Role:
We are seeking obesity medicine physicians to provide longitudinal telehealth care for patients with obesity, and to collaborate with advanced practice professionals and registered dietitians towards exceptional team-based care. Work is remote, and schedules can be flexible.
We are currently accepting applications for aModified FTE (32hrs/week) Obesity Medicine Physician opportunity, who are licensed in AL, TN, OK, MO, IN, SC or GA.
This role will include participation in a rotating after hours call schedule with Form Health clinical leadership and will have a reduced clinical schedule the weeks of on-call coverage.
This is a Modified Full Time position based in a remote, telemedicine clinical setting. The physician will work closely with a supportive care team and have protected time for administrative duties.
What you will do:
- Provide direct patient care in a clinical setting, focusing on evidence-based obesity treatment and chronic weight management. The physician will have their own panel and will collaborate with APP colleagues and Registered Dietitians.
- Review charts and conduct return visits for APP patients every third visit to ensure quality and continuity of care.
- Provide treatment planning and medical management support for complex cases or escalated concerns.
- Prescribe non-controlled obesity medications for patients under both direct care and in coordination with APPs.
- Ensure clinical documentation, protocols, and treatment pathways are followed consistently within the team.
- Assist in the professional development of APPs by fostering clinical growth, offering feedback, and supporting continuing education. This role could include APP collaboration and supervision responsibilities.
- Participate in team-based care initiatives and contribute to a culture of collaboration and high-quality care.
- Attend relevant departmental meetings and contribute to quality improvement efforts within the obesity medicine program.
About You:
- MD or DO degree with board certification; must be an American Board of Obesity Medicine (ABOM) diplomate
- Are licensed to practice and prescribe legally in AL, TN, OK, MO, IN, SC or GA.
- Currently hold a LOQ (Letter of Qualification) with the IMLC (Interstate Medical Licensure Compact).
- Are credentialed with insurance payers (strong preference for multiple state licenses!)
- Have excellent interpersonal skills for patient and team interactions
- Are committed to the highest quality of medical care and an evidence-based approach to obesity management
- Have prior experience conducting telemedicine visits and who have a great "web-side" manner
- Have prior experience working collaboratively with advanced practice professionals and registered dietitians
Bilingual candidates with medical fluency in Spanish are highly preferred

Base Annual Salary
$210,000—$225,000 USD
